Endoskopik retrograd kolanjiyopankreatografi hastalarında geleneksel Türk askeri müziğinin ağrı, anksiyete ve yaşam bulguları üzerine etkisi

Abstract

Bu çalışmanın amacı, Endoskopik Retrograd Kolanjiyopankreatografi (ERCP) hastalarında, işlem öncesi dinletilen geleneksel Türk askeri müziğinin işlem sonrası ağrı, anksiyete ve yaşam bulgularına (sistolik ve diyastolik kan basıncı, kalp hızı, SpO2) etkisinin incelenmesidir. Bu randomize kontrollü deneysel çalışma, 1 Mart 2021- 30 Mayıs 2021 tarihleri arasında Afyonkarahisar Sağlık Bilimleri Üniversitesi Sağlık Uygulama ve Araştırma Merkezi Genel Cerrahi Kliniği, ERCP ünitesinde yürütülmüştür. Çalışmanın örneklemine ilgili tarihler arasında ERCP yapılan 72 hasta dahil edilmiştir (36 müdahale ve 36 kontrol). Veriler "Demografik Bilgi Formu" ve "Hasta Takip Formu" ile toplandı. Hastaların ağrı düzeylerini Visual Analog Scale (VAS) ve anksiyete düzeylerini Visual Analog Scale (VAS-A) anksiyete kullanılarak değerlendirilmiştir. Verilerin değerlendirilmesinde sayı, yüzdelik, ortalama, standart sapma ve ortanca (minimum-maksimum), Kolmogorov-Smirnov Ki-kare testi, bağımsız iki örnek t testi, Friedman testi, Wilcoxon İşaret testi, ANOVA, Mann Whitney U testi kullanıldı. Çalışma bulguları incelendiğinde; müdahale ve kontrol grubu hastaların ERCP sonrası anksiyete puanları arasında istatistiksel olarak anlamlı bir fark olduğu bulunmuştur (p=0,002). Müdahale grubunun ERCP sonrası 2. saat ağrı skorlarının kontrol grubundan anlamlı olarak daha düşük olduğu belirlenmiştir (p=0,033). Yaşam bulguları (SKB, DKB, kalp hızı ve SpO2) bakımından iki grup arasında anlamlı bir fark olmadığını saptandı (p>0,05). Araştırmada elde edilen bulgular sonucunda ERCP yapılacak hastalara işlem öncesinde geleneksel Türk askeri müziği dinletilmesi, hastaların ERCP sonrasındaki anksiyete ve ağrılarını azaltmada olumlu etkilerinin olduğunu ortaya koymaktadır. Bu nedenle, ERCP öncesi kültürel uyum içerisinde olan hastalara geleneksel Türk askeri müziği dinletilmesi önerilmektedir. The aim of this study was to examine the effects of Traditional Turkish Military Music (Janissary Band) music listened to before the procedure on patients' post-procedure pain, anxiety and vital signs (systolic and diastolic blood pressure, heart rate, SpO2) undergoing Endoscopic Retrograde Cholangiopancreatography (ERCP). This randomized controlled trial was conducted in the ERCP unit of the General Surgery Clinic of Health Practice and Research Center at Afyonkarahisar Health Sciences University between March 1, 2021, and May 30, 2021. The study sample included 72 patients who underwent ERCP between the dates the study was conducted (36 in the intervention and 36 in the control groups). The data were collected with the "Demographic Information Form" and "Patient Follow-up Form". The Visual Analog Scale (VAS) and Visual Analog Scale (VAS-A) were used for the evaluation of the pain and anxiety levels of the patients, respectively. Number, percentile, mean, standard deviation and median (minimum-maximum), Kolmogorov-Smirnov test, Chi-square test, independent two-sample t-test, Friedman test, Wilcoxon Signed test, ANOVA and Mann-Whitney U test were used for the evaluation of the data. When the study findings were examined, a statistically significant difference was found between the anxiety scores of the intervention and control group patients after the ERCP (p=0.002). The pain scores of the intervention group patients in the 2nd hour after ERCP were determined to be significantly lower than those in the control group (p=0.033). No significant difference was found between the two groups in terms of vital signs (SBP, DBP, heart rate, and SpO2) (p>0.05). The findings obtained in the study revealed that listening to Traditional Turkish Military Music (Janissary Band) music before the procedure had positive effects in reducing the anxiety and pain levels of the patients who would undergo ERCP. Therefore, we recommend having patients listen to traditional Turkish military music before their ERCP. Keywords: ERCP, Traditional Turkish Military Music, Janissary Band, Pain, Anxiety, Vital Signs
