Yüksek riskli hastada interskalen blok uygulanması

Abstract

Brakiyal pleksus; üst ekstremitelerin duysal, motor ve sempatik innervasyonundan sorumludur. Üst ekstremitenin ve omzun anestezisi brakiyal pleksusun farklı yerlerden bloke edilmesi ile sağlanabilir. İnterskalen blok yan etkisi az, uygulanması kolay, başarı oranı yüksek bir işlem olduğundan ve postoperatif analjezi sağladığından omuz ve üst ekstremite cerrahilerinde kullanımı giderek yaygınlaşmaktadır. Özellikle yüksek riskli hastaların günübirlik anestezi uygulamasında sık tercih edilen bir anestezi yöntemidir. Bu olgu sunumunda, daha önceki ameliyatında kardiyak arrest öyküsü olan, omuzdan malign kitle eksizyonu nedeniyle ultrason ve stimplex eşliğinde levobupivakain kullanarak interskalen blok uyguladığımız hasta sunulmuştur.

Brachial plexus is responsible for sensorial, motor and sympathetic innervations of the upper extremity. The upper extremity and shoulder anaesthesia can be improved by blocking different places of the brachial plexus. Applying interscalene block is becoming common because of its less side effects, high success rates and easy using. This anaesthetic method is often preferred especially in high-risk patients and outpatient anaesthesia. In this case report, we present a patient undergoing malignant tumour excision on her shoulder with a history of cardiac arrest at previous surgery and was applied interscalene block with sonography and stimplex using levobupivacaine.
